Patient Information

Our aim is to ensure that everyone we treat experiences exceptional levels of service

First Visit

For your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment.

The initial exam

Will last approximately one and half hours. This visit will help us learn about your goals for your dental treatment and allow us to outline your existing dental problems and proposed treatment. Generally, X-rays will be taken in order to examine your jaw’s structure, tooth position and decay.Oral hygiene instructions and suggestions will also be provided at this appointment.

We request 2 business days notice for changes or cancellation of your appointment. We are aware that unforeseen events sometimes require missing an appointment, and appreciate your cooperation.Failure to provide 2 business days notice may result in a charge

Dental emergencies

Call our office as soon as possible if you break or loosen any of your appliances. Please do not come directly to the office – by calling us, you will allow us to create a time to see you. Even if you have a regular appointment scheduled, call us immediately to notify us if you need an appliance repaired.

Our office is committed to helping you maximize your insurance benefits. Insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services and we will receive the payment from the insurance company for you. If you have any questions, our staff is always available to answer them.
